[Polymicrobial lung infection associated with idelalisib administration].

Idelalisib es un inhibidor selectivo de la fosfatidilinositol 3-quinasa p110δ, cuya señalización es hiperactiva en muchas leucemias y linfomas de células B1,2. Se han documentado infecciones graves y mortales con idelalisib asociadas a Pneumocystis jirovecii (NPJ) y Citomegalovirus (CMV). La Agencia Española de Medicamentos y Productos Sanitarios (AEMPS) en base a las conclusiones del Comité para la Evaluación de Riesgos en Farmacovigilancia Europeo ha establecido recomendaciones de uso tras realizarse la reevaluación europea del balance beneficio-riesgo del fármaco (Nota informativa AEMPS (7/2016)). Es preciso administrar profilaxis contra NPJ durante el tratamiento con idelalisib, manteniéndola 2-6 meses tras la interrupción del mismo, además de realizar regularmente una valoración clínica y analítica de infección por CMV, especialmente en pacientes con serología positiva o con antecedentes de infección. Los pacientes con viremia deben ser estrechamente monitorizados, debiéndose valorar la suspensión del tratamiento en caso de aparición de sintomatología clínica y mientras no se resuelva la infección3,4.
